certify that my child is in good physical condition and can participate in the
activities involved in the NCFA programs.
I understand that the league will provide only supplemental accident
insurance. I will not hold the Recreation commission, its staff, or other
program participant responsible for accident or injury resulting from my
child’s participation in the program. I
also grant permission to managing personnel or other league representative to
authorize and obtain medical care from a licensed physician, hospital or medical clinic should my child
become ill or injured while participating in league activities when parents are
not available to grant permission for emergency treatment.

WILL AGREE TO RETURN ALL EQUIPMENT ISSUED TO MY CHILD IN GOOD CONDITION, AS WAS
RECEIVED WITH THE EXCEPTION OF
CODE OF ETHICS:
I
understand that my child and his/her family will represent and encourage good
sportsmanship by demonstrating positive support and respect for all players,
cheerleaders, coaches, fans and officials at every game and practice. I understand coaches, umpires, or recreation
volunteers will not tolerate any violation of this ethic and may result in the
removal of my child from participating in the league. I understand fully what
is expected from my family and friends while participating in the NCFA
activities.
PARENT’S PLEDGE:

I will encourage good sportsmanship by demonstrating
positive support for all players, coaches, and officials at every game,
practice or other youth sports events.

I will require that my child’s coach be trained in
the responsibilities of being a youth sports coach and that the coach upholds
the Coach’s Code of Ethics.

I will demand a sports environment for my child that
is free from drugs, tobacco and alcohol and will refrain from their use at all
youth sports events.

I will remember that the game is for youth – not
adults.

I will do my very best to make youth sports fun for
my child.
·
I will ask my child to treat other players, coaches,
fans, and officials with respect regardless of race, sex, creed or ability.

I will help my child enjoy the youth sports
experience by doing whatever I can to assist with making the program better.

I will insist that any of my family and friends that
attend events adhere to these standards as well and will assist by informing
them immediately about any inappropriate conduct.

I will report any misconduct immediately to the
league administrator so that the matter can be addressed and corrected as soon
as possible.
